Requires children to present written documentation of a physical examination performed within 12 months before attending seventh grade, beginning with the 2016-2017 school year.

Physical examinations must be completed by a licensed physician or advanced practice registered nurse.

Allows provisional school attendance if documentation shows the child is undergoing a physical examination, with completion required within three months of first attendance.

Maintains existing requirement for children attending school in Hawaii for the first time to present results of a physical examination performed within one year of attendance.

Effective date is July 1, 2015.
