Extends the sunset date of the Nursing Facility Sustainability Program from December 31, 2015 to December 31, 2016, continuing the collection and use of nursing facility sustainability fees.

Maintains that at least 88 percent of sustainability fee revenue must be used to match federal Medicaid funds for enhancing capitated rates to Medicaid managed care health plans serving nursing facilities.

Allows 12 percent of fee revenue to be used for other departmental purposes, with remaining moneys distributed back to nursing facilities by December 30, 2016.

Appropriates $12,000,000 from the nursing facility sustainability program special fund for fiscal year 2015-2016 for program purposes.

Exempts the nursing facility sustainability program special fund from certain state central service assessment and administrative expense requirements.
