Establishes an address confidentiality program within the crime victim compensation commission to provide substitute addresses for victims of domestic violence, sexual offenses, or stalking who have relocated.

Program participants receive substitute addresses for use on public records while maintaining confidentiality of actual addresses; the program receives and forwards mail on behalf of participants at no charge.

Requires state and local government agencies to accept and use substitute addresses when presented with valid program authorization cards, except for voter precinct designation, property taxes, motor vehicle registration, and real property records.

Creates a $28 surcharge on convictions for stalking, domestic violence crimes, sexual offenses, and related criminal attempts; 95% of collected surcharges fund the address confidentiality program surcharge fund.

Program participants must be certified by a victim advocate or victim service provider and certification lasts four years; participants may withdraw or renew certification, with grounds for cancellation including failure to update information or submission of false information.
