Requires public schools to provide annual health information to parents or guardians of sixth grade students beginning with the 2016-2017 school year.

Information must include details about human papillomavirus (HPV), its connection to cervical and other cancers and diseases, and availability of HPV vaccines.

Information must cover tetanus/diphtheria/pertussis (Tdap vaccine), meningococcal disease (meningococcal conjugate vaccine), influenza (annual vaccine), and other CDC-recommended preteen vaccinations.

Requires notification that the Advisory Committee on Immunization Practices recommends HPV vaccination be completed before students enter seventh grade.

Department of Education, in collaboration with the Department of Health, must develop and approve all information materials prior to dissemination by schools.
