Establishes disclosure and notification requirements for entities or persons using more than a specified threshold of restricted use pesticides annually, effective July 1, 2016.

Requires posting of pesticide warning signs at least 24 hours before outdoor application and throughout the restricted-entry interval, with specific information including trade name, active ingredients, hazards, and emergency numbers.

Mandates pre-application written notification at least 24 hours in advance to properties within a specified distance of the application area containing pesticide details and safety information.

Prohibits outdoor pesticide applications in buffer zones around sensitive areas (schools, hospitals, care facilities, shorelines, watersheds, and bodies of perennial waters) unless a single-use exemption is granted by the department of health.

Requires annual public reporting within 60 days of year-end to the departments of health and agriculture detailing all pesticides used, quantities, and geographic locations by tax map key and ahupuaa.
