Requires the Department of Public Safety to begin collecting data on July 1, 2016 from offenders entering the correctional system regarding their parental status and number of children under age eighteen.

Mandates collection of additional information about incarcerated parents and their children that the department deems useful for facilitating services to this population.

Directs the Department of Public Safety to collaborate with the family reunification working group and other stakeholders to develop intake forms and questions needed to obtain the required information.

Requires the department to develop a plan for managing collected data and ensuring public disclosure in accordance with applicable laws.
