Director of Health is requested to convene a task force to assess the scope and feasibility of establishing a centralized filing system for advance care planning forms accessible by all health facilities in Hawaii.

Task force shall focus on centralized filing for health care power of attorney, provider's orders for life-sustaining treatment, advance health care directives, and other forms deemed relevant by the Director of Health.

Director of Health or designee serves as task force chairperson; Director selects members including representatives from hospitals, health care facilities, nursing homes, hospital administration attorneys, and the University of Hawaii Elder Law Program.

Task force must submit findings, recommendations, and any proposed legislation to the Legislature no later than twenty days before the 2016 Regular Session convenes.

Task force is dissolved on June 30, 2017.
