Appropriates $4,800,000 for fiscal years 2015-2016 and 2016-2017 to restore basic adult dental benefits to medicaid enrollees, with the Department of Human Services to obtain maximum federal matching funds.

Appropriates $800,000 for fiscal years 2015-2016 and 2016-2017 for outreach and eligibility services at federally qualified health centers to help children, families, and individuals enroll in medicaid and children's health insurance programs.

Appropriates $1,000,000 from the community health centers special fund for fiscal years 2015-2016 and 2016-2017 to establish health homes for medicaid enrollees at federally qualified community health centers under the Affordable Care Act.

Appropriates $1,000,000 in general revenues for fiscal years 2015-2016 and 2016-2017 to provide administrative support for the health homes program.

Appropriates $6,000,000 for fiscal years 2015-2016 and 2016-2017 to the Department of Health for direct primary medical, dental, and behavioral health care for uninsured and underinsured patients at community health centers, distributed on a per-visit or per-member per-month basis.
