Authorizes health care providers under Hawaii's state medicaid program to place a question on medicaid certification and re-certification forms asking "Are you currently participating in the Supplemental Nutrition Assistance Program (SNAP)?"

Allows medicaid providers to collect and release aggregate SNAP participation data to the legislature without including individual names and subject to federal privacy regulations and HIPAA requirements.

Addresses findings that 159,603 people enrolled in medicaid in October 2015 were likely eligible but not participating in SNAP, potentially leaving $247,000,000 in unclaimed federal funding.

Aims to increase SNAP participation by raising awareness among eligible low-income individuals and families about program benefits and availability through outreach by health care providers.

Takes effect upon approval.
