Requires physicians, osteopathic physicians, physician assistants, advanced practice registered nurses, and naturopathic physicians to provide a current electronic mail address when applying for or renewing a license.

Licensees must notify the licensing authority of any change in email address within 30 days; email addresses may only be shared with other state or federal agencies for public health and safety purposes.

Amends disease reporting requirements to mandate that laboratory directors include complete demographic information (name, date of birth, residential address, and phone number) when reporting individuals affected by or suspected of communicable or dangerous diseases to the Department of Health.

Expands the scope of health care professionals subject to disease reporting penalties, changing language from "refuses or neglects to give notice" to "violates this section," with fines up to $1,000 per violation.

Takes effect July 1, 2016.
