Establishes an Address Confidentiality Program administered by the department of accounting and general services to protect victims of domestic violence, sexual offenses, or stalking by providing substitute addresses for use in public records.

Allows eligible individuals (adults, parents/guardians of minors, or guardians of incapacitated persons) to apply for the program upon recommendation of a victim advocate or service provider, with applications requiring evidence of victimization and a statement that the applicant has or will confidentially relocate.

Requires state and local government agencies to accept and use a program participant's substitute address on public records when presented with a valid authorization card; establishes special rules for voter registration (using actual address for precinct designation but substitute address for mailings) and exempts property tax, motor vehicle, and real property records from using substitute addresses.

Creates a mail forwarding system whereby the program receives and forwards first-class, certified, and registered mail sent to participants' substitute addresses at no charge to participants.

Establishes an $28 surcharge on convictions for stalking, crimes involving domestic violence or sexual offenses, and attempts/conspiracy/solicitation thereof, with 95% of surcharge proceeds funding the Address Confidentiality Program Surcharge Fund and 5% supporting crime victim compensation; effective July 1, 2050.
