Requires students to present written documentation of a physical examination performed within 12 months before attending seventh grade, beginning with the 2016-2017 school year.

Allows provisional school attendance if a student submits documentation that a physical examination is in process, with completion documentation required within three months of first attendance.

Maintains existing requirement that children attending school for the first time in Hawaii must present physical examination results from a licensed physician or advanced practice registered nurse performed within one year.

Appropriates unspecified funding from general revenues for fiscal years 2015-2016 and 2016-2017 to cover recordkeeping, record review, accountability, and parent notifications related to mandatory seventh-grade physical examinations.

Effective date is July 1, 2050.
