Amends section 386-25 of Hawaii Revised Statutes to establish that vocational rehabilitation providers must submit complete initial evaluation reports to the employer and director within 45 days of referral or selection.

Requires initial evaluation reports to include assessment of employee's medical status, primary and secondary disabilities, unrelated disabilities, and physical or psychological limitations.

Adds a penalty provision stating that providers failing to file a complete report within the prescribed timeframe are subject to section 386-97.5 penalties.

Takes effect on July 1, 2050.
