Appropriates $1,000,000 for fiscal years 2015-2016 and 2016-2017 from the community health centers special fund to establish health care homes for medicaid enrollees at federally qualified community health centers.

Appropriates $4,800,000 for fiscal years 2015-2016 and 2016-2017 from general revenues to restore basic adult dental benefits to medicaid enrollees.

Appropriates $800,000 for fiscal years 2015-2016 and 2016-2017 from general revenues to provide outreach and eligibility services for medicaid and CHIP enrollment at federally qualified health centers.

Requires the Department of Human Services to solicit and accept the maximum available federal matching funds for all three appropriations.

Effective date is July 1, 2015.
