Requires the Department of Public Safety to collect data upon intake of offenders into the correctional system beginning July 1, 2016, including the number of offenders who are parents and the number of their children under age eighteen.

Directs the department to collect any additional information about incarcerated parents and their children that the department deems useful to facilitate services to these populations.

Requires the Department of Public Safety to collaborate with the family reunification working group and other stakeholders to develop intake forms and questions needed to gather the required information.

Mandates the department develop a plan for management and public disclosure of collected data in accordance with applicable laws.

Takes effect July 1, 2020, with the intent to identify children impacted by incarceration and support service providers in accessing federal funding for assistance programs.
