Allows beneficiaries in counties with 3,000+ square miles of land area to use non-network retail community pharmacies if they live 10+ miles from the nearest network pharmacy, provided a non-network pharmacy exists within 10 miles of their residence.

Prohibits pharmacy benefit managers and prescription drug plans from imposing higher copayments, quantity limits, refill restrictions, or other administrative requirements on beneficiaries using non-network pharmacies compared to network or mail-order pharmacies.

Requires non-network retail community pharmacies to be reimbursed at $12.50 dispensing fee plus product cost at the higher reimbursement rate.

Increases maximum fines for pharmacy benefit manager violations from $10,000 to cover violations of the new geographic exemption provisions, with authority for the insurance commissioner to order corrective action and restitution.

Requires the insurance commissioner to submit annual reports to the legislature through 2021 regarding the measure's effects on healthcare costs, including impacts to the Hawaii employer-union health benefits trust fund.
