Requires students entering seventh grade beginning with the 2017-2018 school year to provide written documentation of a physical examination performed within twelve months prior to attendance.

Physical examinations may be performed by a licensed physician, physician assistant, or advanced practice registered nurse.

Department of Education must notify parents or guardians of the requirement when their child enters sixth grade and post the requirement on the department's website.

Department of Health shall contact parents or guardians of seventh-grade students who do not submit physical examination documentation by December 31 and provide assistance in obtaining an exam.

Department of Health shall provide follow-up consultations to parents or guardians about healthy lifestyles, preventative health care, and health insurance coverage options under the Affordable Care Act or Medicaid, subject to available funding.
