Prohibits discrimination by all health insurers in Hawaii based on actual or perceived gender identity in participation and coverage decisions for individual and group health insurance policies.

Defines discrimination to include denying or limiting insurance based on gender identity, charging differential premiums based on gender identity, treating gender identity as a preexisting condition, and denying coverage for gender transition-related services or sex-specific medical services.

Requires medical necessity determinations for covered services to be applied consistently regardless of gender identity and establishes that copayments, deductibles, and coinsurance must be no less favorable than those for other covered medical services.

Applies to all health insurance types including accident and health policies, hospital and medical service policies, health maintenance organizations, and state employee health benefits plans under Chapter 87A.

Takes effect January 7, 2059.
