Establishes the Hawaii kupuna trust fund as a separate fund within a 501(c)(3) nonprofit entity selected by the director of health with the governor's concurrence to provide grants strengthening elderly and vulnerable adult care.

Fund assets consist of state, county, and federal appropriations; private contributions from corporations, businesses, and individuals; and investment income, with a requirement that state appropriations not supplant existing elderly and vulnerable adult program funding.

Creates a seven-member advisory board including representatives from the senate, house, department of human services, executive office on aging, and members appointed by the governor and the selected nonprofit entity to develop strategic plans and grantmaking criteria.

Authorizes grants for elder assistance including prevention of abuse and financial exploitation, fall injury prevention, transportation and mobility assistance, aging and disability resource center promotion, and training and education for caregivers and professionals.

Takes effect July 1, 2050.
