Authorizes healthcare providers under Hawaii's Medicaid program to include a question on certification and recertification forms to determine applicants' housing status, with the question to be composed by the Department of Human Services.

Allows Medicaid providers to collect and release aggregate housing status information to the legislature without including individual names.

Requires that any information released comply with federal Medicaid privacy standards and the Health Insurance Portability and Accountability Act of 1996 (HIPAA).

Establishes a data collection system to gather information on homeless individuals' usage of publicly funded medical assistance programs to inform policy decisions on resource allocation.

Takes effect on January 1, 2017.
