Establishes an address confidentiality program within the crime victim compensation commission to provide substitute addresses for victims of domestic violence, sexual offenses, or stalking to keep their actual residential addresses confidential.

Allows eligible applicants (adults, parents/guardians of minors, or guardians of incapacitated individuals) to apply through a victim advocate or service provider and receive an authorization card with a substitute address valid for four years.

Requires state and local government agencies to accept and use substitute addresses on public records when presented with a valid authorization card, with exceptions for voter precinct designation, property taxes, motor vehicle registration, and real property documents.

Establishes procedures for agencies to request disclosure of actual addresses through a formal review process, with expedited procedures available for criminal justice officials and protections against disclosure during discovery or court proceedings unless found necessary by the court.

Creates the address confidentiality program surcharge fund through a $28 surcharge on convictions for stalking, domestic violence-related crimes, sexual offenses, or attempts/conspiracies thereof, with 95% of funds dedicated to program administration.
