Requires pharmacy benefit managers providing services to Hawaii residents to register with the Insurance Commissioner before operating in the state.

Applies to entities performing pharmacy benefit management services including mail service pharmacy, claims processing, formulary development, rebate contracting, and disease management programs.

Establishes a nonrefundable application fee of $140 for initial registration and annual renewal fee of $140 due by March 31 each year.

Imposes a $500 fine for each violation by any person operating as a pharmacy benefit manager without valid registration.

Authorizes the Insurance Commissioner to adopt rules under chapter 91 to implement and enforce the registration and regulatory requirements.
