Establishes a Homeless Incentive Program administered by the Department of Human Services in conjunction with the Governor's Coordinator on Homelessness to encourage chronically homeless individuals to use shelter services.

Provides $12 per 24-hour period to chronically homeless individuals for each overnight stay in a shelter, contingent only on shelter space availability and individual compliance with shelter rules.

Defines "chronically homeless individual" as a homeless person with a disability who has lived in places not meant for human habitation for at least 12 months continuously, or on at least 4 separate occasions in the last 3 years totaling 12 months.

Requires the Department of Human Services to adopt rules to implement the program and appropriates unspecified general revenue funds for fiscal years 2017-2018 and 2018-2019.
