• Requires health insurance policies in Hawaii to cover embryo, oocyte, and sperm cryopreservation procedures for cancer patients diagnosed before starting treatment.

• Covers one lifetime procedure for adult females (either embryo or oocyte cryopreservation) and one lifetime procedure for adult males (sperm cryopreservation).

• Includes all outpatient expenses for the procedures such as evaluations, laboratory assessments, medications, treatments, and cryopreservation costs, but excludes ongoing storage fees and future medical costs related to using the preserved genetic material.

• Requires patients to sign a legal directive addressing extraction, preservation, designated guardianship of genetic material, posthumous use rights, and rights of resulting offspring before undergoing the procedure.

• Applies to accident and health insurance policies, hospital and medical service contracts, and health maintenance organization plans; effective July 1, 2017.
