Extends the Hospital Sustainability Program through June 30, 2019, adding two additional fiscal years (2017-2018 and 2018-2019) beyond the previous June 30, 2017 expiration date.

Updates the definition of "net patient service revenue" to use Medicare cost report data from three years prior to the applicable state fiscal year instead of the fixed 2013-2014 fiscal year.

Increases the cap on hospital sustainability fees from three percent to three and one-half percent of net patient service revenue for both inpatient and outpatient care services.

Appropriates $70,000,000 from the Hospital Sustainability Program Special Fund for each of fiscal years 2017-2018 and 2018-2019 to be used by the Department of Human Services.

Modifies the definition of "private hospital" to include hospitals that became private operations in calendar years 2017 or 2018 (previously limited to those in operation in calendar year 2015).
