Requires all health insurance policies in Hawaii to include coverage for cancer treatment for firefighters, with fees reimbursed at usual and customary rates comparable to major health plans.

Provides firefighters with 100% average weekly wages as disability indemnity if cancer causes permanent or temporary total disability, with benefits paid promptly without waiting for director decision.

Establishes service-connected disability retirement and death benefits for firefighters with one or more years of service who develop cancer after a required pre-employment physical examination.

Exempts cancer cases from the workers' compensation system under chapter 386, creating a separate benefit process; effective July 1, 2050.
