Amends Hawaii Revised Statutes Section 574-1 to eliminate requirements that married persons or civil union partners change their names upon marriage or civil union.

Allows each party to a marriage or civil union to declare any middle or last name they choose to use, with no requirement for spouses or partners to have identical names.

Removes previous restrictions limiting name choices to names legally used in the past by either spouse or partner, allowing complete freedom in selecting any middle or last name.

Takes effect on January 7, 2059.
