Requires limited service pregnancy centers to distribute written notice informing clients about Hawaii's free or low-cost family planning and pregnancy-related services available through public programs like Med-QUEST, including online application information.

Defines "limited service pregnancy center" as facilities advertising prenatal sonography, pregnancy tests, or pregnancy options counseling that are not comprehensive health care facilities, and excludes hospitals and federally qualified health centers from the definition.

Establishes privacy protections requiring centers to obtain written authorization before disclosing any individually identifiable health information and mandates free written pregnancy test results be provided immediately after testing.

Allows individuals to request and examine their health records within 15 working days and requires centers to provide free copies upon request.

Authorizes civil penalties of $500 for first violations and $1,000 for subsequent violations enforceable by the Attorney General, and allows private civil actions for damages up to three times actual damages plus attorney's fees.
