Requires the Department of Human Services, in collaboration with the Department of Health, to review the Hawaii Pathways Project pilot and continue pursuing Medicaid funding for supportive housing services for chronically homeless individuals.

Directs the department to consider eligibility criteria, supportive housing services (including outreach, housing search assistance, eviction prevention, and coordination with health providers), and application for a state 1115 Medicaid waiver amendment when planning expansion of services.

Requires the Department of Human Services to submit a report to the legislature no later than twenty days before the 2018 regular session describing the status of the Medicaid waiver amendment, proposed supportive housing services, and eligibility criteria.

Appropriates unspecified general revenue funds for fiscal year 2017-2018, contingent on dollar-for-dollar federal matching funds, to be expended by the Department of Human Services for purposes of this Act.

Takes effect July 1, 2050.
