Extends the Hospital Sustainability Program from June 30, 2017 to June 30, 2019, and extends related statutory provisions to December 31, 2019.

Updates net patient service revenue calculations to use Medicare cost report data from three years prior to the relevant state fiscal year instead of a fixed fiscal year 2013-2014 baseline.

Expands the definition of "private hospital" to include hospitals that became private in calendar year 2017 or 2018 and are currently operating.

Authorizes the department to use hospital sustainability fee revenues and federal matching funds to enhance capitated rates paid to Medicaid managed care health plans for fiscal years 2017-2018 and 2018-2019.

Appropriates $65,000,000 from the Hospital Sustainability Program Special Fund for each of fiscal years 2017-2018 and 2018-2019 for the Department of Human Services.
