Establishes the "End of Life Option Act" allowing terminally ill adults with capacity to make medical decisions to request a prescription for medication to self-administer to end their life.

Requires a terminal disease diagnosis confirmed by two physicians (attending and consulting), with a six-month prognosis, plus two oral requests at least 15 days apart and one written request signed by two witnesses.

Mandates mental health specialist assessment if attending or consulting physician identifies signs of mental disorder before any prescription can be issued.

Prohibits coercion or undue influence; allows withdrawal or rescission of request at any time; protects health care providers, pharmacists, and persons present from civil or criminal liability for good faith participation.

Requires Department of Health to collect anonymized data and publish annual reports on prescriptions written, deaths from aid-in-dying drugs, and demographic information about individuals who used the option; effective July 1, 2017.
