Requires health insurance policies in Hawaii to cover embryo, oocyte, and sperm cryopreservation procedures for cancer patients diagnosed with cancer who have not started treatment.

Coverage limited to one lifetime procedure per patient: one embryo or oocyte cryopreservation for adult females, or one sperm cryopreservation for adult males.

Covered costs include all outpatient expenses for the cryopreservation procedures such as evaluations, laboratory assessments, medications, treatments, and cryopreservation itself, but excludes ongoing storage costs and future pregnancy attempt costs.

Applies to accident and health insurance policies, hospital and medical service contracts, and health maintenance organization plans, with procedures required to conform to American College of Obstetricians and Gynecologists or American Society for Reproductive Medicine standards.

Patients must enter into a legal directive addressing extraction, preservation, posthumous use, and rights of any offspring resulting from in vitro fertilization before undergoing the procedures; effective July 1, 2017.
