Prohibits health insurance entities from imposing preexisting condition exclusions on individual and group accident and health insurance policies, hospital and medical service plan contracts, and health maintenance organization policies issued or renewed in Hawaii.

Prohibits health insurance entities from charging higher premiums or contributions based on an individual's gender compared to a similarly situated individual of the opposite gender under the same or substantially similar policy.

Applies nondiscrimination protections across multiple insurance regulatory frameworks: individual and group accident and health insurance (Chapter 431), hospital and medical service plans (Chapter 432), and health maintenance organizations (Chapter 432D).

Defines preexisting condition exclusions broadly to include any limitation or exclusion of benefits based on conditions present before the effective date of coverage, including conditions identified through pre-enrollment questionnaires, physical examinations, or medical record reviews.

Effective date: July 1, 3000.
