• Establishes a midwife licensure program administered by the Department of Commerce and Consumer Affairs requiring licenses for anyone practicing midwifery or using the title "Licensed Midwife" in Hawaii.

• Sets qualifications for two types of licensed midwives: Certified Midwives (CM) with full prescriptive authority and Certified Professional Midwives (CPM) with limited authority to obtain and administer specific non-controlled legend drugs.

• Requires triennium license renewal with proof of current national certification, continuing education (10 contact hours in pharmacology), and documentation of any disciplinary actions or criminal convictions.

• Allows the director to deny, suspend, or revoke licenses for grounds including fraud, incompetence, unprofessional conduct, impairment while practicing, and violation of licensing laws.

• Creates a six-member advisory committee (three midwives, one obstetrician-gynecologist, two public members) to assist with implementation and establishes penalties of up to $1,000 per offense for violations.
