Requires health care providers to obtain written informed consent from patients (or parents/guardians for minors) before administering each vaccination.

Mandates health care providers gather and provide complete individual and family health history, vaccine manufacturer's product insert sheet, and benefits and risks information including the CDC's Vaccine Information Statement.

Defines "informed consent" as written voluntary agreement to receive a vaccination and "health care provider" as licensed physicians, advanced practice registered nurses, physician assistants, or authorized department of health representatives.

Maintains existing requirement for immunization records to be kept by providers and made available to the department of education and department of health.

Takes effect upon approval.
