Requires each county to establish a registration system for medical cannabis collectives to regulate and tax the distribution of cannabis for medical use, with at least one functional collective per county by January 1, 2019.

Defines "distribution" as the transfer of cannabis between a primary caregiver and qualifying patient, or between qualifying patients, with reimbursement allowed for expenses such as time, fertilizer, soil, and electricity costs.

Changes the legal defense for qualifying patients and primary caregivers from an "affirmative defense" to a "defense" under Chapter 329 and Chapter 712 medical cannabis laws.

Allows qualifying patients to transfer medical cannabis between themselves without legal penalty.

Appropriates funds to Hawaii's four counties (Hawaii, Kauai, Maui, and Honolulu) to establish and administer medical cannabis collectives for fiscal year 2018-2019 (dollar amounts not specified in the bill).
