Expands mandatory insurance coverage for in vitro fertilization (IVF) procedures to include same-sex couples, unmarried women, and male-female couples where male infertility is the relevant factor.

Removes the requirement that oocytes be fertilized with a spouse's sperm, instead allowing coverage when sperm from the insured or insured's spouse is used in IVF procedures.

Adds coverage for expenses of oocyte donors and surrogates used in IVF procedures for the insured or insured's dependent spouse.

Reduces the required infertility history from five years to twelve months and adds same-sex couples as an alternative qualifying condition for coverage eligibility.

Takes effect on July 1, 2017, and applies to both individual and group accident and health insurance policies and hospital or medical service plan contracts that provide pregnancy-related benefits.
