Requires licensed home and community-based case management agencies to report all referrals made by the agency to the Department of Health, including whether any referral fees were paid and the amounts received.

Mandates the Department of Health to publish and annually revise an online listing of all licensed home and community-based case management agencies on its official website, including referral and fee information.

Adds a new section to Chapter 321, Hawaii Revised Statutes establishing reporting requirements for case management agencies serving community care foster family homes, expanded adult residential care homes, and assisted living facilities.

Amends Section 321-482 to clarify Department of Health authority over home and community-based case management agencies and establish standards for licensing, evaluation, and oversight.
