Requires the Department of Health to collaborate with the Department of Human Services and Department of Human Resources Development to develop a single, unified action plan to reduce diabetes incidence in Hawaii and improve diabetes care.

Mandates a biennial report to the legislature no later than 20 days before each regular session in odd-numbered years detailing diabetes impact, affected populations, program participation, and associated costs.

Report must include assessment of diabetes education and treatment effectiveness, coordination between departments, recommended legislative action steps with proposed legislation, and updated information on implementation needs and resources.

Limits initial requirements to compilation of existing diabetes information and data unless departments have unencumbered funding for diabetes prevention that can be used for new research and data collection.

Takes effect July 1, 2017.
