Establishes a Medical Aid in Dying Act allowing mentally competent adult residents with a medically confirmed terminal disease (six months or fewer to live) to request and self-administer prescription medication to end their life in a humane and dignified manner.

Requires two physician confirmations (attending and consulting provider) of the terminal diagnosis, patient mental competence, and voluntary request, with psychiatric evaluation if deemed necessary.

Mandates multiple safeguards including an initial oral request, written request witnessed by two people (one unrelated to the patient), a 15-day waiting period between requests, and a 48-hour waiting period before dispensing medication; patient can rescind the request at any time.

Provides immunity from civil, criminal, and professional liability for healthcare providers and facilities acting in good faith compliance with the act; allows facilities to prohibit participation by notifying providers in advance.

Amends Hawaii Revised Statutes to exclude actions taken under the Medical Aid in Dying Act from prohibitions on assisted suicide, mercy killing, and murder charges; establishes criminal penalties (Class A felony) for forging requests, coercing patients, or destroying rescission documents.
