Extends the nursing facility sustainability program through December 31, 2019 (previously set to expire December 31, 2017)

Requires at least 88% of nursing facility sustainability fee revenue to match federal Medicaid funds for enhancing capitated rates to Medicaid managed care health plans, with 12% available for other departmental purposes

Changes the reference period for state appropriations baseline from June 30, 2016 to June 30, 2018 for purposes of determining whether to discontinue fee collection

Appropriates funds from the nursing facility sustainability program special fund for fiscal years 2017-2018 and 2018-2019 to sustain Medicaid payments to private nursing facilities

Maintains program termination conditions if federal approval is denied, funding is reduced, money is misused, or federal financial participation becomes unavailable
