Requires all individual and group health insurance plans providing pregnancy-related benefits to include mandatory coverage for in vitro fertilization (IVF) procedures on a gender-neutral basis available to same-sex couples, unmarried women, and male-female couples regardless of which partner has infertility issues.

Reduces the infertility history requirement from five years to twelve months and expands eligibility to include same-sex couples as an alternative qualification criterion for IVF coverage.

Extends coverage to include expenses for oocyte (egg) donors and surrogates used in IVF procedures, enabling coverage for same-sex male couples and other scenarios requiring donor eggs or surrogacy.

Clarifies that IVF benefits cover only expenses directly related to in vitro fertilization services and exclude other pregnancy-related or post-IVF outpatient services.

Exempts the Medicaid program from the mandatory IVF coverage requirements and takes effect on July 1, 2070.
