Directs the director to develop a consolidated application form that combines the motor vehicle driver's license application (Section 286-111) and the civil identification card application (Section 286-303).

The consolidated form applies to all instruction permits, provisional licenses, driver's licenses, limited purpose driver's licenses, limited purpose provisional driver's licenses, and limited purpose instruction permits.

Amends Section 286-303(c) to reference the director's consolidated form developed pursuant to Section 286-111 for identification card applications.

Maintains existing fee structures and verification requirements for all applicant types, with fees determined by each county council.

Takes effect on July 1, 2050.
