Amends Hawaii Revised Statutes § 574-1 to replace restrictions on name changes upon marriage or civil union with expanded flexibility for parties to choose any middle or last name.

Eliminates the requirement that married persons or civil union partners must select names from those legally used in the past by either party, allowing adoption of any middle or last name upon marriage or civil union.

Establishes that no person shall be required to change their middle or last name upon entering into a marriage or civil union.

Removes the requirement that parties to a marriage and partners in a civil union have the same middle or last name.

Takes effect upon approval.
