Extends the nursing facility sustainability program through June 30, 2021 (previously set to expire June 30, 2019) to continue supporting long-term care facilities in Hawaii.

Increases the maximum nursing facility sustainability fee from 4 percent to 5.5 percent of net patient service revenue, with per-resident daily fees capped at $20 for most facilities and $9 for reduced-fee facilities (up from $13.46 and $5.85 respectively).

Allows at least 88 percent of fee revenue to be used either to match federal Medicaid funds for enhancing capitated rates to managed care health plans or to pay quality incentives, while 12 percent may be used for other departmental purposes.

Requires Medicaid managed care health plans to make payments to nursing facilities within 30 calendar days after receipt of monthly capitation rates from the department.

Appropriates $21,000,000 out of the nursing facility sustainability program special fund for each of fiscal years 2019-2020 and 2020-2021.
