Exempts service provider agencies from licensing requirements when providing services exclusively to participants in the Medicaid home and community-based services (HCBS) intellectual and developmental disabilities waiver under Social Security Act section 1915(c), provided the agency is approved by the Department of Human Services Med-QUEST Division.

Maintains existing exemption for agencies under contract with the City and County of Honolulu elderly affairs division and adds the new Medicaid waiver exemption as alternative grounds for exemption.

Makes permanent the licensing exemption provisions by repealing the June 30, 2019 sunset date that was previously set in Act 21, Special Session Laws of Hawaii 2009.

Allows agencies serving Medicaid waiver participants to avoid nursing certification requirements while maintaining oversight through continuous quality assurance and monitoring by the Department of Health's Developmental Disabilities Division and Med-QUEST Division.

Takes effect January 1, 2059.
