Requires the Department of Human Services to establish a Medicaid buy-in program for workers with disabilities under federal Social Security Act section 1902(a)(10)(A)(ii)(XIII).

Establishes eligibility requirements including net countable income exceeding 138% of federal poverty level, disability status under federal law, and net nonexempt resources within specified limits, with disability benefits exempted from countable income.

Implements sliding-scale premiums ranging from $20 to $250 per month based on countable income, with annual adjustments subject to legislative reporting 20 days before each regular session.

Authorizes the department to contract with public or private entities for premium collection and allows discontinuation of eligibility for nonpayment of premiums for more than two months.

Requires the Department of Human Services to submit a report to the legislature by 20 days prior to the 2021 regular session evaluating implementation and viability of a full Medicaid buy-in program.
