Amends Hawaii Revised Statutes Section 346-378(e) to replace the definition of "chronically homeless individual" with a new definition aligned with the 2015 U.S. Department of Housing and Urban Development federal definition.

Defines a chronically homeless individual as a person with a mental illness, addiction, or physical disability who lives in a place not meant for human habitation, a safe haven, or emergency shelter and has been homeless continuously for at least 12 months or on at least 4 separate occasions in the last 3 years (totaling at least 12 months with minimum 7-day breaks between occasions).

Includes individuals who have resided in institutional care facilities (jail, treatment facilities, hospitals) for fewer than 90 days and met chronic homelessness criteria before entering, counting such stays as part of the 12-month total rather than breaks in homelessness.

Extends the definition to include families with an adult head of household (or minor head of household if no adults present) who meet the criteria, including families whose composition has changed during homelessness.

Effective date set for December 31, 2059.
