Establishes a dementia screening and awareness pilot program administered by the Hawaii Department of Health to promote screenings statewide with emphasis on Native Hawaiian and uninsured populations.

Requires the program to provide public education on dementia screening, collaborate with health care providers to integrate screening into standard practice, and partner with providers to screen eligible participants.

Limits eligible participants to individuals at least 50 years old (or younger with high dementia risk), uninsured or underinsured, not eligible for medical assistance, and with gross family income at or below 250% of the federal poverty level.

Appropriates $400,000 for fiscal year 2019-2020 and $400,000 for fiscal year 2020-2021 to fund the pilot program.

Requires preliminary and final reports to the legislature on program expenditures and becomes effective July 1, 2019, with automatic repeal on June 30, 2021.
