Extends the deadline for counties to adopt a surcharge on state tax ordinance from March 31, 2019 to June 30, 2021.

Adds two new implementation dates for county surcharges: January 1, 2021 for ordinances adopted between March 31, 2019 and June 30, 2020, and January 1, 2022 for ordinances adopted between June 30, 2020 and June 30, 2021.

Maintains the requirement that counties must conduct a public hearing and notify the director of taxation within ten days of adopting a surcharge ordinance.

Preserves the existing restriction that no county surcharge on state tax may be levied before January 1, 2019 or after December 31, 2030.
